ST Therapy

Physical Therapy Intensive Treatment Summary of Progress

__________ participated in a 3-6 week intensive program receiving Occupational, Physical and Speech Therapy, as well as LiFT and Microcurrent Technology. At the onset of the intensive treatment program, __________ had solid gross motor skills of 36 months with several areas lacking consistency and quality; with some scattered skills up to 48 months. The chart attached illustrates his progress. The final day of treatment, _________ had achieved a solid gross motor level of 48 months with significantly more quality and consistency; with some scattered skills up to 60 months. He achieved 2 years of progress over the course of his program.

The results below indicate an overall summary of his Occupational Therapy progress achieved during that time. Over the course of the 3-6 week program, __________ gained 24-month jump in postural stability, 12 month jump in fine motor/visual motor skills; 24 month jump in self-care/adaptive behavior skills; a 12-month jump in sensory skills; and a 24-36 month jump in praxis/bilateral coordination skills.
