COVID-19 Protocol For Patients

IT4K is committed to your health and safety. We have adopted the following patient protocols for our center:

• Wash hands before coming in.
• Remove shoes and place in bin.
• The therapist will take your temperature and record it on a chart.
  • If your temperature is above 99 degrees, the appointment will be rescheduled.
• When in the hallways and lobby please wear a face mask.
• Practice social distancing – stay at least 6 feet from other people.
• If you are uncomfortable in any way, please let us know so we can address it.
• Please NO siblings in sessions.

Also, quick questions for you…

1. Have you traveled domestic or international in the last 30 days?
2. Have you or your family experienced flu like symptoms in the past 2 weeks?
3. Have you been exposed to anyone who has tested positive for COVID-19?
